Our Mission
To be everyone’s “First Call” to help guide patients, families and caregivers who find themselves in difficult health situations through mobile primary care.
Christi Barnas
Christi joined First Call Medical Concierge to establish a community-based organization that provides primary care wherever you call home. We saw a gap in the healthcare continuum and are striving to fill this need to provide supportive healthcare to patients and their families. After a life changing experience with her grandparents receiving hospice care, Christishifted gears as an RN in ER medicine and began her work in Hospice and Palliative medicine, where she worked to maintain comfort and quality of life for chronic and terminally ill patients. She has outstanding credentials with a BSN, a BA in Philosophy, a Master of Science in Nursing, a member of the American Association of Nurse Practitioners and the California Association of Nurse Practitioners. She also served as President of the Sacramento chapter of the Hospice and Palliative Nurses Association from 2018-2020. She also worked delivering preventive care services to migrant farm workers and their children in a farming community in southwest Georgia as part of the Farm Worker Family Health Program.Â
Shontel Forcier
Shontel Forcier joined First Call Medical Concierge Team as our Program Manager/MA. She has extensive experience in the medical field most recently as a Phlebotomist and Lab Manager. Home healthcare is very near and dear to her heart as one of her family members has had to have different levels of home care her entire life, which has made such a difference in her family’s life. She is excited to impact and provide similar services to those in need and continue her medical experience in the Medical Concierge world and dive deeper into helping the community. She is currently a member of NSNA (National Student Nurses Association) through Sacramento City College and is working towards becoming a Registered Nursing, with a goal to pursue a bachelors, and becoming a nurse practitioner one day. Shontel, along with the entire First Call Medical Concierge team, is excited to impact the community and will continue to develop programs to support patients through all phases of adult life.

Primary Care
Our practitioners become your main healthcare provider. A primary care provider is central to your health success.

Chronic Care Management
We participate in Medicare’s Chronic Care Management program, or CCM. The purpose of CCM is to help patients with chronic conditions better manage and optimize their health.

Transitional Care
Whether you are in a rehabilitation/skilled nursing facility, assisted living or just getting back to your home, we will be there for in-person or telehealth appointments. Transitional Care helps to prevent hospitalizations by spending more time with patients as they move along the spectrum of care.
